The broken reference is the table object, but there is only the replacement image from the table object the object itself does not really exist in the document. So trying to edit the table activates another object from duplicate pair. Removing of one of the object leaves a zombie object that connects to any newly inserted object, this is why removing did not help. Looks pretty like a duplicate to the issue 79302. At least there seems currently to be no known way to get such a broken document after the fix. And now ( in OOo2.4 ) the document should be correctly repaired, that means that one the two references that point to the same object gets a new object with the same content. As result there is no more object duplication in the document. But the lost object ( the table in this case ) can not be repaired since it does not exist in the document already.
