In OOo3.0RC4 On WinXP,the issue exists. I personally guess it is a duplicate 
of issue 75173 .

In fact, ALL fractionnal formats with a quoted string does not work e.g

#+??/?? gives the same result 1 00/81

Applying the fractional format
  # ??/?? 
to the value 
  12345.6789 
gives 
  1 00/81 

Correct result is 12345 55/81. The problem lies with the quoted space. 
The format # ??/?? gives the correct answer.

NB: The format # ??/?? is used by Excel with French locale
