The behavior is correct, because the manual format of the Index heading will be
overwritten when updating it. Thus it does not belong to the outline anymore.

right-click into Heading of alphabetical index, choose "Edit paragraph style".
Now go to "Outline & Numbering" tab in that dialog and select "Level 1" for
Outline level.

Actually I changed the type associated to the Title of the Alphabetic Index into
Heading_20_1 in the Style pan and the title returned to the general index. I am
not clear if that is what you meant, but it works :-)

Actually I have no queue as to how I got in that situation; the interaction
between the two indexes is quite tricky to me.

As to your gentle suggestion, I am not clear what you mean by adding an
attribute, assuming I need to do it to the field 'Intestazione Indice Analitico'
in the Italian version, the translation is not obvious.

p.s. I extended the platform, I tried it at home on my mac and problem remained.

The alphabetic index title has paragraph format "Outline level 1", but the index
is write-protected.
question is: how did you get the attribute there? It is a "hard" (or direct
attribute) fornat which is overwritten every time the index is updated.

To workaround this problem, add the attribute in the paragraph style "Index
heading".

I attach the file. To replicate the error it is enough to refresh the Alphabetic
Index and than the General Index to the see the header of the former
disappearing from the latter.
A simple refresh of the General Index keeps the header, thus pointing to a bug.
