We just have a accidental new case and found that the user placed some textframes(anchored to the page)and a picture also anchored to the page on Page2 where the textcursor has ad that moment no access (because there is no text placed on page2)and the user only started typing on page 2. MRU-MST: enter a page break in page one. Curiously this generates two Undo steps.
