I have the same problem.

I just can't make the page numbers in my document's index into hyperlinks.

Also, there is a problem of multiple occurrences of one word on the same page
(i.e.: the cause of the dreaded p symbol which I always disable). One solution
is to make clicking on page number hyperlinks to move the cursor to the first
occurrence of the word on the page in question.
