2008 ---
Form controls inside page header/footer are not supported yet.
First investigations reveals that the code for changing the anchor of a form
control does not consider, this restriction. This causes the crash.

Workaround: Place the form control inside the page text area before changing its
anchor type to to-paragraph

MRU-OD: insert a control in the margin above the upper text border. Change its
anchor to page. then insert insert a page header. change anchor of control to
paragraph and then to character - crash.
BTW: crashed even in OO 1.1.

2. set control's anchor value with to page
3. select menu - insert - header - default
4. select the control and set its anchor value with to paragraph or to 
charactor

actual result:
OpenOffice will stop and then crash.
