How about putting the disclaimer information in the Instructions box? Then make the last question a check box: I have read the Instructions at the top. Hi, Mo. If they have to acknowledge the information, then it really can't be read only, right? You can have a question (your disclaimer) and an answer (their acknowledgement) but then not do anything with it. You don't have to map it to the back end fullfillment form.
