How about putting the disclaimer information in the Instructions box?

Then make the last question a check box: I have read the Instructions at
the top.

Hi, Mo. If they have to acknowledge the information, then it really can't be 
read only, right? You can have a question (your disclaimer) and an answer 
(their acknowledgement) but then not do anything with it. You don't have to map 
it to the back end fullfillment form.

Has anyone been successful in finding a way to provide a read-only question
to the end user on an SRD? I know that it might not make sense but this is
the situation we have. We have few questions on an SRD and we need to
include a text field with a disclaimer for the customer to read it and
confirm acknowledgement by checking a check box. The only caveat is that the
