[AZORES-Genealogy] Baptismal and death records 1957 to 1960

Where can I find my brother's death certificate with the date's above. Not sure 
when he passed away. He was a baby and my Mother doesn't remember due to age 
and sickness. I need help. He was born in Finais Deluz.  I don't know what 
hospital he died in,nor does he have a headstone. His name was Joao Alberto 
Soares. 

Please help.
