 bacula.  The online docs don't seem to have it.

Also sometimes you'll need to take the tape offline first if you want
to do any changer operations outside of bacula.

mt -f /dev/st0 offline 

usually helps when mtx won't play nice (insert your appropriate tape
device above).

OK, this gets me one step closer.   Honestly I have not found a good

At least on Linux you could use lsscsi -g to find the devices. The  
medimux is the changer. Next is to use /dev/tape/by-id/ instead  
/dev/sgX in the Bacula config with the matching device ID, so no  
trouble with udev.
