The prceeding shows *3* SCSI controllers: scsi0, scsi1, and scsi2.

The one you want is scsi2, which is an LSI 53C1030

Hit the LSI site, and see what's what there - it may need a custom driver,
or a bit of a reconfiguration, or summat. The last bit about "more
targets are available" implies something is not configured.

Also, the tape is at id 6 lun 0, the changer is at id 6 lun 1, which means
the changer *should* be /dev/sg6... There's something niggling at the
back of my mind about having to do something with some devices control
file to enable the other LUN - but it won't come to the front.

Hmm... a bit outdated, that system :-) Can you suggest an upgrade to a
more recent version? The one you have is a CentOS 4.x, I believe the
most recent CentOS 4 has 2.6.9-89 or newer. You might also benefit
from some security related fixes.

Always challenging to discover them when you're already out of space :-)

F/W bug sounds like firmware bug... Have you checked if you can
upgrade either the HBA's firmware or the kernel module?

That looks ok - one disk and one backplane at scsi0.

This is the RAID-enabled LSI HBA (IIRC, PERC HBAs use LSI chips).

On the RAID HBA scsi1, it's two volumes and the enclosure services.
Looks ok, too.

Tape drive and autochanger connected, looks good. Running on their own
HBA, which is also good. You should, as pointed out, verify that the
SCSI bus they are connected to is not configured as RAID.

This is disturbing. Without looking into this, this message sounds
like an aborted SCSI transaction, due to a firmware issue.

I would recommend to verify that the SCSI buses are in the proper
modes - the one connecting the autochanger should not be set to RAI

No, it's not attached to to a PERC, but it still could be a RAID controller.

The LSI 53C1030 is a dual channel Ultra 320 controller chip and as such
cold be used as a straight controller, (my system is using an 53C1020 -
single channel version without problem.

However, there are RAID controller products using the 53C1030:

so you will need to check.

How about sg3_utils, then? Try:

   sg_map -i -x

It is in the CENTOS pools: yum install sg3_utils
and is installable as static binaries. I could send you a static sg_map,
if you want (and you trust me enough to run it...).

The changer is at:
/dev/sg5

The tape drive on this changer is at:
Data Node /dev/st1
No Rewind Node /dev/nst1

The tape drive and changer did work under Bacula when the system was
running Fedora Core 3. Since I had to upgrade to RHEL 5 I have had
issues getting Bacula running but that has been just me being dumb.

Hope this helps.

Dan, is the changer attached to one of the PERC controllers (which I
believe are RAID controllers)?

If so, the note in the above mentioned Dell doc (SCSI Bus Requirements)
  may be the answer:

"NOTE: The autoloader may not work with multiple SCSI LUNS when attached
to a redundant array of independent disks (RAID) controller. The
autoloader is not recommended for use with a RAID controller. If this
problem occurs, it is recommended that the autoloader be attached to a
separate SCSI bus controller on the server."

Bugger. I get none of the above:

*sigh*

I'd like to help these people out, but well, there *seems* to be a
hardware issue. Their admin, now departed, claims backups to tape were
working. Backups to disk are working, but not tape. Nothing in
bacula-sd.conf refers to any tape drive. Nothing in the command history
refers to the tape drive.

They have 8x400GB volumes on disk and they're out of space.

Mysteries.

Make that easily 5 or 10 minutes if the library somehow has to inventory
its content, because a tape magazine has been extracted and reinserted
for instance.

I have the very same library on a centos host (Dan's guess regarding the
changer device name looks right). They are awfully slow when it comes to
moving tapes. If it has to bring a number of them in front of its
barcode reader, you'll have to wait before a "mtx status" or "update
slots" works.

Does the failure remain after retrying after a minute or two? I've used
some changers that tended to throw errors like the above for the first
half-a-minute or so following a reset. Waiting a minute was required
for the changer to settle back to normal.

Nice catch.

Sorry, I don't know anything about the physical unit. I'm told it's a
Dell PowerVault PV-124T and the mtx output supports that claim.

But they are different LUNs, which should be fine AFAIK.

Just a wild guess, but are the drive and the changer a single physical
device with only one interface and scsi address setting? Or can you somehow
set the id separately for them? Now both have id=6, might it help if you
