Sometimes (less than 1 time per 3 months) I see this with my archive.
And the solution is to stop bacula-sd. If that did not work unload mt

modprobe -r mt

If that did not work unload the scsi driver then reload it.

for me this is sym53c8xx

so

modprobe -r sym53c8xx

modprobe sym53c8xx

usually does the trick.

So I see that but it seems to me like maybe there should be an entry for
mt for the tape drive too then? I have since rebooted both the tape
loader and server in hopes of fixing the problem to no avail. I wish I
could get the tape drive working so we can get our back ups running.

i couldnt get the tape i was using out of the drive and our drive is at the 
back of the machine so i had to fiddle with it to get it to move the tape back. 
i am using a pretty up to date kernel and in the process of trying maybe to 
call support on the errors i get running btape test i updated the firmware on 
the tape autoloader which in turn reboots the loader. i did just check the 
dmesg using dmesg | grep scsi and i grepped mt both didnt return anything of 
significance. scsi had a message about ch0 not getting established. since 
getting the tape unjammed i am rerunning btape test will see if it works.

mt -f /dev/nst0 status

If so this is really bad. It is essential that mt works with your tape drive

Does

mt -f /dev/st0 status

work?

If not do any of the following work:

mt -f /dev/nst1 status

or

mt -f /dev/st1 status

Can you post the output of

mt -f /dev/nst0 status

to get the status of the tape drive.

I think this got side tracked I know now why the status doesn't work on the 
tape drive but only on the autochanger. However I am trying to trouble shoot 
the tape drive because I cant get it working with bacula. I run the btape test 
which errors out and looks like;

17-Sep 11:25 btape JobId 0: Error: block.c:568 Write error at 0:0 on
device "Ultrium 4-SCSI" (/dev/nst0). ERR=Input/output error.
17-Sep 11:25 btape JobId 0: Error: Error writing final EOF to tape. 
This Volume may not be readable. dev.c:1681 ioctl MTWEOF error on   
"Ultrium 4-SCSI" (/dev/nst0). ERR=Input/output error.
btape: btape.c:827 Error writing block to device.

When I run the status on the changer I show that in my case that the 1st slot 
is empty where a tape was placed and slot 2 the other slot I placed a tape is 
full and shows the tape barcode number. I can only assume the tape is in the 
drive.

Is there something in the bacula manual on how to turn on logging in the
mtx-changer script, how do I do that?

Device {
   Name = "Ultrium 4-SCSI"

Look at the line:

Archive Device = /dev/sg0


It is likely that it should be something like:

Archive Device = /dev/nst0

If that isn't it, then enable logging for the mtx-changer script and see
what the log 
file says about the failed load.  You can test with "mount" and
"release", rather 
than attempting to "label".

With lsscsi I never find a device for /dev/nst0 is why I went with
/dev/sg0 is it possible bacula is expecting the latter and resolves it
on its own somehow?

This is the output from lsscsi -g
