Try to modify the mtx-changer script and add a sleep 10 after the mtx load command. To add to this, apparently this tape is just bad or needs to be erased. I marked it read only and mounted another tape into drive 0 and everything seems to be working. With /dev/st0 there is a chance of corruption caused by the os rewinding the volume without bacula expecting.
