That is, you aren't placing your pages_controller.php in the same place on the filesystem as the standard pages_controller.php. You are placing yours in app/controllers/pages_controller.php. In fact, you can copy the standard pages_controller.php to that location, and use it as a starting point. Based on what Felix describes in that post, I typically do the following:

That gets cake included into your script. From there you would require
and instantiate your Controller, and go about business as usual, making
sure that you're not depending on data being passed as it would in a web
app, unless you are going to set $_GET and $_POST appropriately
yourself, or manually pass input to your Controller action invocations.
For instance, here's a snippet of how I use CakePHP inside of phing to
handle some build tasks that actually live inside of a Controller. Note
that init_acl and init_auth are the names of the actions/methods defined
in my BuildController (and I store BuildController in my app/vendors
directory rather than app/controllers):

A process question for core developers: how are fixes to 1.x.x.x ported to 1.2.x.x

I don't believe you have to make changes to RequestHandler --
$this-RequestHandler-isAjax() will still work, as it checks that he
request was made via an XMLHttpRequest.
As for the existing $this-ajax-link(), I believe something like the

Or conversly, would it be better to make a method that I could run via an URL that would immediately disable their application. Something whereby I would have to login to an account that I have on all installations, run it, and disable their program until they paid. Is this more logical? Are you Zend Guard'ing the source code for the application? What's to keep a customer from removing the code that implements the software lysine deficiency on their end? I considered this factor several years ago when I implemented a payment system in Perl that did some creation of XML data before sending info to the credit card processor. My answer to the question was to move a large portion of the application code to my server, which I then called via XML-RPC, and which I released and installed on the client's server once the client paid (which they did). This way, the client could not remove the call-home functionality because calling-home was the only way the app could function properly. This however, added another layer through which semi-sensitive data had to travel (not credit card numbers, but unique identifiers that related customers to items they selected for purchase). It also created a dependency on the stability of my systems, which is okay if one can ensure that stability. There is debate as to the utility and legality (in certain instances) of software lysine deficiencies. IANAL, obviously, but perhaps you can get away with it as long as it's noted in your license agreement that you reserve the right to disable application functionality for non-payment. This sort of scheme seems more feasible, and poses less of a security vulnerability, than having an account on each installation which you must login to to disable the application. That valid_login entry gets set from my app/controllers/users_controller. When a user tries to hit a protected section of my application (which is all parts of my app), app_controller's beforeFilter redirects them to /users/login unless that valid_login entry exists in my cake session. This /users/login action is where I present a form requesting username and password, when there is no post data, or verify the posted username and password as valid.
