Bugzilla has been around since I can remember - maybe even years !? ...it is Netscape/Mozilla's bug tracking tool - it is huge and complicated - way too much for the average joe. - and a bitch to get it working on Windows. Another vote for Mantis. Its easy to install, easy to use and purdy powerful.
