We use Superscout by Surfcontrol, quite cheap, easy to configure, plus good
reporting tools.  Worth a look

You can use Cisco cache engines. These machines have Rules Template option
and you can filter url's by defining patterns.

Well, for the authentication part you can use the aut
proxy feature (both router IOS and PIX) coupled with a
RADIUS/TACACS server. Local user db will work to. 

As far as blocking porn, etc there are other products
like WebSense but thats the only one that works with
the PIX at the moment. You could redirect the web
traffic to a proxy box (wccp, transparently, etc) and
have proxy check with a blacklist service (web sense,
etc) as well.
