Do you have a User statement in clamav.conf which is lower than root?
If so you need to ensure that user has permission to create/remove files in 
/var/clamav,
(ls -ld /var/clamav to check).

 I'm trying to get the clamav-milter to work with sendmail.  I've made all
 the required changes to the sendmail.cf file, but when I try to restart
 sendmail, I get the error:
 sendmail: WARNING: Xclmilter'': local socket name
 /var/clamav/clmilter.sock' missing.

 I've verified, and the clmilter.sock file is indeed in the
 /var/clamav/clmilter.sock directory (srwxr-xr-x1 root root
 0 Mar  3 16:51 clmilter.sock).

 Here is some info about the system:
 Redhat 8.0
 Kernel 2.4.18
 Sendmail 8.12.5-7 (though when connecting via telnet, the version is
 8.12.8/8.12.5)
 ClamAV version 0.67-1

 I've compiled ClamAV with the --enable-milter option, and it works fine.
 clamd starts up fine as well, and all tests seem to go through.  One thing I
 noticed is that when I execute /usr/sbin/clamav-milter -blo
 /var/clamav/clmilter.sock, I get a warning: /usr/sbin/clamav-milter:
 running as root is not recommended.  However, I can see via ps that it is
 running.

 Is there something I missed?  Any help is appreciated.

give the clamav user the ownership;
chown clamav:clamav /var/clamav/clmilter.sock

Did you intend to say 'and the clmilter.sock file is indeed in the
/var/clamav directory?  If so, the only time that I have run into this when
the uid for the clamav-milter process did not have authority to create the
socket.

doesn't sendmail need WRITE access to that socket?  unless you're 
running sendmail as root (not a prime idea), it won't have write access 
to that object.

I don't think it needs write access to the socket file itself, but it 
does need read+exec privs to the directory containing the socket at 
least afaik.
