A quick read of the clamav-milter man page suggests you missed the
--server option:

--server=HOSTNAME/ADDRESS, -s HOSTNAME/ADDRESS
  IP  address  or  hostname of server(s) running clamd (when using
  TCPsocket and --external).  ...
