We have experienced the issue fairly recently as well. Maybe it will match the
OP's setup. The scenario we encountered it was a cluster of 3 nodes running
3.11.4 with a secondary index constructed using Stratio's Cassandra Lucene
plugin ([https://github.com/Stratio/cassandra-lucene-index]). By mistake, we
ended up having a really long value inserted into a particular column (which
was well past the ability for Statio to index) but were trying the use the
value of that to locate similar records. Like the example above, this ended up
getting the node where the query was run into a sick spot where it couldn't
recover without a restart.

Can you share the query you issued to get into this state? It's clearly some
kind of read, and filter is large (or referencing something large).
