Maybe it will match the OP's setup. The scenario we encountered it was a cluster of 3 nodes running 3.11.4 with a secondary index constructed using Stratio's Cassandra Lucene plugin ([https://github.com/Stratio/cassandra-lucene-index]). By mistake, we ended up having a really long value inserted into a particular column (which was well past the ability for Statio to index) but were trying the use the value of that to locate similar records.
