OK, it took many attempts but finally I reproduced this with nvidia. Turns out it's not a window stacking problem at all. I don't know how they got in the state, but two applications (Thunderbird and TortoiseHG) got very stuck in fullscreen mode (not just maximized, but 100% of my LCD). No amount of F11, maximize, moving, rebooting, etc. would unstick them.
