I suppose you run 12.10 natively, ie with unity-3d and Compiz. The root cause is that update-manager can change the executable or libs that compiz depend on and a page fault can bring an inconsistent segment into memory. We don't have a good way to protect against that. We're not guaranteed that restarting compiz would work, since that may trigger this page fault. Besides compiz rdeps that rely on its ABI need to be upgraded in the order for it to succeed. Unity is such an example. As a workaround post-precise, we may try to use something like mlockall to mount all pages in memory upon the reception of a signal to protect compiz from such a scenario. And then restart. The code for this plugin had not been thoroughly tested and enabling it without enough user feedback was considered risky. There were a lot of compiz issues at the time. This one had a lower priority unfortunately. Sam confirms btw that there is no pre-requisite for this plugin. And problably caused by an incomplete upgrade or an ABI problem. Can you double check the version of compiz installed, and even the actual compiz executable and plugins being used. Ok, so this sounds like an another HW limitation problem, where the addition of a second head prevents the chipset to drive all screens that the initial resolution and results in poor results. Intrisically it is a driver bug: the driver shouldn't force the HW to go beyond its limits.
