I too have had nearly *exactly* the same issue and setup as you have, but
haven't heard anything else on this list about it. I've lost two NT Server
installs at work over it before I gave up, and nearly lost my partitions at
home. I get the same error: { An error occured mount failed: No such
device } every time. I know the partitions I am attempting to create on the
drives are NOT an issue, as I have used RedHat 5.x and 6.x, and Mandrake
7.02 (ranging from Fips to Draxto create them from scratch and install
successfully before.
I sorely wish someone would let us know we're not going crazy, and that
there is some sort of identifiable issue here

I have two slightly different cenarios to report.
First on my system at home, I have two primary dos paritions,
and an exteneded partition I use for Linux.
The first partition is a fat16 dr-dos part. The seocond is a fat32 win 98
part.
The first time I installed , I changed re-labled the mount points for the
two M$ partitions, and then proceeded to the formating.
The first thing that was odd was that the list of partitions to be formated
included my fat16 partition. This is wrong, it should not show up in the
list.
Second when the ext2 partition formating was complete, I got the following
message:
{ An error occured mount failed: No such device }
I could not get further in the install process.
Later on I retried the install, and everything worked fine.
Second My machine at work:
I have two drives, The first has two fat16 dos partitions and an ntfs
partition,
The second drive is an etended partitioin drive I use for linux.
Similar thing happens on my machine at work,
The ntfs partition shows up in the list of partitions to be formated,
and after the ext2 formating is complete, I get the same error as above:
{ An error occured mount failed: No such device }
