AFAIK, intellimouse is always with wheel. So choosing it should work for you.

use "PS/2|Generic PS2 Wheel Mouse" or "USB|Wheel"

This sounds like what happens when gpm is still enabled. Try disabling gpm.

The MS Intellimouse?  I don't think so.  I have two at my office plugged
into PS/2 ports (on two different computers, that is).
