Given the lack of documentation on SRB, all I can do is submit a report in the
hope of getting clarification.

Basically, sorting and grouping does not seem to work.

The effect I want is:

First sort on a "collation" field.

Then sort & group on another "type" field.

The result seems to have no particular order.

I would attach a copy of the database here but there doesn't seem to be a way of
doing it (unless it's in a later page)

Could you be more specific on the version of OpenOffice.org you are working 
with. Version 1.0.0 is quite old and I suspect that this was just an oversight 
on your part. The current stable release is 3.0, could that be the version you 
are using.

Also, which Linux distro you are using?
Are you using the OpenOffice.org application supplied by your distro, or 
downloaded from the OO.o website?
Finally - the type of database you are working with. Is it an embedded Base 
database vs MySQL?

OK - there error is real it seems, however it also seems to you could avoid the 
error.

It appears that you are using an ODB file for the address data.
In this case what you should be doing is registering the ODB file with 
OpenOffice.org. This registration process allows you to have a common name for 
the datasource associated with the correct path on the individual users 
machines.

When you distribute a new form then, the use of the Registered Datasource name, 
versus the hard coded path to a file should avoid this proboem totally.
