I have the same problem. I've tried to change the odbc driver with the latest
(july 20th) but nothing changed. I have also tried to use ODBCng driver from
commandprompt inc but it is not in the list of drivers available (in the
registry it has a new entry, not the same of others ODBC drivers and the key 
has no settings).

Regression, it is working in OOo 3.1, and not in m55.
See attached odb file, open form.
The fields in left side bounded to the same varchar_ignorecase type field, and
in right side controls to the same varchar field. Two-two control to the same
field. 
If you enter data into text fields (lower row controls), that case all numbers
stored in table. 
If you enter numbers into formatted fields(upper row controls), the data not
inserted into table. 

I found it when tested issue 101135. Phone numbers stored in text field and
inserted through formatted fields.

Well christoph,  thats easy

attached you get the sample-dbf (DPARA1)

However, you may be more interested in the attached picture,
and you will see real fun:
- the original DBF in a viewer - the firstfield is a recordcounter - value 1000
- the file open in OO.o the field is formated as 19.2 but displayed as original 
- store it under OO.o as a copy: the header ist stored correct, but the content
is stored wrong (1000.00)
- reopen the "stored under" version displays right, although the content is 
wrong! 

Make it simple: OO.o produces confusion with DBF-files, and at actual it is
nearly impossible to reopen the "stored as"-DBF-files from OO.o in applications
other than OO.o

give it a try, and you will see what I mean: The cursor is behind the spaces

This issue comes after a discussion on the issue 103371
(http://www.openoffice.org/issues/show_bug.cgi?id=103371).
The names of the events in a FormDocument in Database seems to be problematic.
For me (a french people who speaks with some difficulty in english), "Document
closed" means that the form is already closed so launching an embedded macro is
a nonsense and "Document is closing" means the document is about to be closed so
an embedded macro can be called.
So in my humble opinion, perhaps the names should be reversed.

configuration : Windows XP, OOo 3.1, MySql database on a remote serveur (not 
localhost)

It is very very slow compared to ODBC Driver (mysql-connector-odbc-5.1.5-
win32.msi).

fs->msc: As said above -in the bug doc, a macro is assigned to the "Document is
closing" event, which is wrong - it must be assigned to "Document closed".
Before the fix, this wasn't called, now it is.

fs->thierrym: Offering the "Document is closing" event (which has a wrong name,
see above) for the document itself is nonsense, strictly, there is no
possibility that this is ever called, since by definition the document is
already dead when it would need to be fired. However, "Document closed" works 
now.
