Hi, I also experienced this 2 weeks ago on a Dual Opteron machine running debian-amd64 (kernel v2.6.11), but only after upgrading to 4.1.12. Running a "show processlist;" query would show several clients making SQL queries but no progress at all. And mysql would not shut down no matter what; Only a kill would do it. After that I downgraded mysql-server-4.1 to version 4.1.11a-4 and it has been running stable and in production ever since. Have you tried this version?
