Did the message already occured the first time you started klatexformula? Could you please try to delete the .klatexformula directory and check if the error message pops up already on the first start? epstopdf reported an error (exit status 2). Please note that the window that should pop up is not an additional window that you can see in e.g. the taskbar, but a pop up note containing the equation. Unless you have a long equation the additional preview window is abut the same size as the build in preview window. There is also a button on the bottom-right, above the large Quit button showing a magnifying glass. Pressing this should also pop up the enlarged preview.
