After updating X to the new X-Org 1.4.0 . the left-handed mouse is no
longer set when starting KDE.

The same behaviour on i386.

I can confirm this. The KDE mouse configuration is simply ignored. 
To have the mouse working as I need it I momentarily added

option ButtonMapping 3 2 1

to the InputDevice section of my xorg.conf
