For me this seemed to be caused by switching from GNOME to a minimal tiling WM desktop. As outlined in a [GitHub Issue][1], the solution for me was to [get Qt to recognize GTK+ Icons][2] by setting an environment variable in ~/.xinitrc as follows:

export DESKTOP_SESSION=gnome

I'm not sure what other effects this variable may have, but the icons are back.
