Hi Lukasz,
could you please execute this command being sure you get the error? It
will log all actions from faxaddmodem into /tmp/faxaddmodem.log. Then
please send to me that file.

# sh -x /usr/sbin/faxaddmodem 2/tmp/faxaddmodem.log
