I'm not able to reproduce this, mysqld_safe is part of the binary
package mysql-server-5.1 in 5.1.45-3:

# dpkg -L mysql-server-5.1 | grep mysqld_safe
/etc/mysql/conf.d/mysqld_safe_syslog.cnf
/usr/bin/mysqld_safe
/usr/share/man/man1/mysqld_safe.1.gz
