The wiki page you mentioned does not really explain how to do so. Is
there a simple, step-by-step, process described somewhere ?

This doesn't contain anything related to a proper Debian source package.
checkinstall is not suitable for the official Debian packages. Please
follow https://mentors.debian.net/intro-maintainers and links on that page
if you want to create and maintain a proper Debian package in Debian.
You'll also need a proper build system instead of shipping the pre-built
binary in the sources.

The main patch looks good, a couple of things needs to be addressed before I 
will sponsor it.

Firstly you need to send a mail with the debdiff attached stating your intent 
to NMU to the bug
report and giving the maintainer and appropriate time (guidelines say 5 days in 
this case) to
respond.

Secondly your version number is weird, I would suggest using 0.2.1-1+nmu3 in 
this case.
