After some further cogitation, I spoke to the fetchmail author, and it turns out that it was a problem with fetchmail. He's fixed it in the next version, soon to be released. Once that's done fetchmail users on sytems running exim shouldn't need the -mda option to fetchmail or any special workarounds in their exim config.
