I set this up with xf86config.  Just tell it you have a:
ps/2 mouse
and that the location is:
/dev/psaux
that's always done the trick for me.

Well, I can think of two situations, that might cause this problem:
1. gpm is not started
2. somehow your gpm.conf gets changed
