Of course the bug can be close, it was because I've recently reinstall all my packages (due to an amd64 migration) but keep my old copy of /var so alternatives were marks has being already installed. I just purged and reinstalled xcursor-themes, it seems to work fine. Ok the update-alternatives --install /usr/share/icons/default/index.theme \ x-cursor-theme /etc/X11/cursors/$THEME.theme had not installed /usr/share/icons/default/index.theme for a strange reason.
