I haven't noticed that happening it in a long time now. The box in
question exhibits other strange problems with X now, so I am starting to
think this may be an X server problem.

hmm - I've not seen (or noticed) anything like that.  There's an
issue with focus that cropped up a while back (from X server changes),
that I've got on my to-do list (after the scrolling issues are all
stabilized).  From the description, this could be either (or none).

Having the application redraw the window is one way to see - another
would be for xterm to redraw the window (using left-control-mouse menu).
That modifies less of xterm's state (especially the selection).
