When setting gnome to display the preferences menu in the title bar, the
icon is shown in a wrong way.

Application icon is displayed too big and not centred within the event
hitbox.

theme.)

Update on my previous comment (#4): I checked what "pam-auth-update
--force" did (it backs up the old versions), and ended up accepting all
its changes so that I will hopefully never have this problem going
forward.  Because it added pam_systemd to common-session I have now
removed the line I added to gdm-launch-environment and gdm3 still works
(even with Wayland).

When I remove all the Default Handlers of an application in gnome-
control-center, I am unable to reset the default handlers to the
original values as the reset button is gone.

I expect the reset button to still be there, so that I can recover the
removed Default Handlers when I need to.

When I try to unset a Default Handler of an application in gnome-
control-center, it removes the item but doesn't update the screen. I
have to click another application and back to that application to see
the results.

I expect that it will update the screen and remove the item that I
unset.

Ubuntu 18.04.2 LTS, latest updates / upgrades as of today.
DELL XPS 13 9350

Problem started immediately after upgrading to 18.04

logout / login workaround does the trick, but problem comes back every
half an hour or so. So Ubuntu not usable on my beloved notebook :-(

The solutions here did not work in my case, but a solution which did work for 
me can be found in

which says:

The file causing the problem is /etc/xdg/autostart/gnome-keyring-pkcs11.desktop 
. The line
OnlyShowIn=GNOME;Unity
should be changed to
OnlyShowIn=GNOME;Unity;XFCE

There are similar statements above, but none include XFCE.

I had a similar issue.  This error in syslog finally lead me to my
solution:

As a result of that error, gdm-x-session fails further along in the gdm3
start up process and aborts:

The gdm3 service just keeps trying to start gdm-x-session over and over again, 
flooding syslog.
I fixed it by adding this line to /etc/pam.d/gdm-launch-environment:

session optionalpam_systemd.so

My theory on the real problem is that pam-auth-update is run during some
package installs and it would have added pam_systemd to /etc/pam.d
/common-session.  However, I had done manual edits to at least one
/etc/pam.d/common-* file, in which case pam-auth-update aborts if "--
force" is not specified.
