this happens after a complete shut down or restart. 
when i double click on a video, (usually an avi file) the screen is maximized 
and no way to get out of that state.

it does not happen all the time. But when it happens, i have to turn off
visual effects to get totem out of full screen mode.

look at bug 158840

* If so, what specific steps should we take to recreate this bug?
turn on visual effects to custom or even extra, 
play an avi file,
under right click menu uncheck show controls option, 
either stop the video play back before closing totem or not. 
shut down or restart,
double click on a video to open then you'll encounter the problem.
