   2. Fill out some of the form fields.

   3. Save the file and reopen it with LibreOffice writer.

   4. Form fields that have been filled out are no longer visible.

  - Updating LibreOffice to 6.4.4 (either from the official site or from 
ppa:libreoffice/ppa fixes this problem.

  - The form data can actually be recovered by opening the file with AbiWord 
(to my best knowledge, neighter Microsoft Word nor the fixed version of 
LibreOffice are able to recover the data).

This does not affect recent versions of Ubuntu since they are shipped
with a newer Libreoffice package.

I noticed the following error messages on the console when starting
evince from the command line:

(evince:14906): IBUS-WARNING **: Unable to connect to ibus: Could not connect: 
Permission denied
(evince:14906): IBUS-WARNING **: Events queue growing too big, will start to 
drop.

It looks like this is related to 
 https://bugs.launchpad.net/ubuntu/+source/evince/+bug/1587968

and the following workaround proposed by Sander van Malssen
(https://bugs.launchpad.net/ubuntu/+source/evince/+bug/1587968/comments/11)
did work for me as well (confirmed with two different machines that have
been upgraded to 16.04).

   rm -r ~/.config/ibus
