I tried to unlock the modem with mmcli and it looks like partially
worked.

mmcli -m  
returned:
'successfully connected the modem'
and the modem led blinking indicates 3G connection.
However, in the settings menu I can still see "Mobile Broadband Off" and no 
Internet works.

When I try to choose "Mobile Broadband Connect" (assuming that the modem
is now unlocked) I get nothing. I also get nothing now when I choose
"Mobile Broadband Settings"

  After inserting USB 3G modem it is visible in the system:

  shows that the model is locked and sim pin is required.
  There are no dialogs open to enable the user to supply the pin.

  In the Broadband connection wizard there is no place to enter the pin and
  the connection creation fails.

My problem was solved by installing package: nvidia-331-updates instead
of nvidia-331.

Similarly for Lenovo   T420 nvidia  NVS 4200M except I can switch to
text mode with cntr+alt+Fx, switch back to intell and restart lightdm. I
can also confirm that switching between nvidia and intell had worked
previously for ubuntu 14.04 however I cannot tell exactly when it
stopped.

I took a fresh install of Ubuntu 14.04 on Lenovo Thinkpad T420 with Nvidia NVS
Mute microphone button works fine. 
Also muting in the sound settings switches the led indicator on the button on 
and off as expected.

I installed nvidia driver 331 required by CUDA toolkit.
Restart.
System works fine except the microphone mute button. 
However, muting the mic in the sound settings dialog switches the led indicator 
correctly again.
It looks like only the button is not sending any event to the sound settings.

Switching to nouveau driver solves the problem but CUDA will not work.
