With current ubuntu LO (4.3.3) the issue stands.

As I wrote upstream, the added text seems to be the control's 'label'
property.

Printing with printer language PDF or exporting to PDF a document with
form elements, the resulting PDF has added further text to the side of
form elements ('checkbox',...).

Exporting to actual PDF forms, this does not happen. The graphical
representation of the elements changes, however.

Printing with printer language PS does not include that additional text
either.

Bug description:
  Printing a libreoffice document containing form elements (checkboxes
  etc.) get superimposed with additional text: e.g. 'Checkbox' besides a
  checkbox etc.

  Switching 'Printer Language Type' to PostScript solves that - that
  setting is, however, not saved and has to be manually re-done
  everytime.

  I suppose there is a difference in that 'postscript' does not really
  contain a checkbox but its visual representation and 'pdf' does
  actually contain a checkbox?
