I think this is now fixed with changes in NetworkManager. I can't reproduce this. It shouldn't even ever happen, there seems to me any items missing from the menu anyway, items that aren't being removed or touched anywhere but together as a group. Please attach the file ~/.xsession-errors, you might be able to see errors there pertaining to nm-applet.
