In the toString() it looked like they provide extra underscores where 
appropriate. So I suppose that the example above would be en__htmlhelp.

excerpt:
Getter for the programmatic name of the entire locale, with the 
language, country and variant separated by underbars. Language is always 
lower case, and country is always upper case. If the language is 
missing, the string will begin with an underbar. If both the language 
and country fields are missing, this function will return the empty 
string, even if the variant field is filled in (you can't have a locale 
with just a variant-- the variant must accompany a valid language or 
country code). Examples: "en", "de_DE", "_GB", "en_US_WIN", "de__POSIX", 
"fr_MAC"

The Open Group (formerly X/Open) uses underscore between
language and country code.  I couldn't find any ISO
standard that defines the whole locale string, just
separate standards for language codes and country codes.

We went through this for daisy.org
I think its - (hyphen) as in
: ISO 639 language code optionally followed by a two letter country code as 
in ISO 3166. For Sweden: "SV" or "SV-SE", for the United Kingdom: "EN" or 
"EN-UK" etc.

The Java platform uses underscore in the java.util.Locale.toString() method and  

has some examples. The JavaDoc at the top of the class says to use the most 
important variant first. Hope this helps.

On UNIX, for specifying character encodings it's

which is very similar.

Is there such a specification for variations?

Cool! Several people pointed me that they need little bit different
localizations for HTML Help than for HTML (omision "Chapter" from
chapter title and so on). This will allow smooth integration with rest
of stylesheets.

Is there a way to have a cascade/hierarchy of gentext modifications like
you can with the templates? For example, if I have a set of core gentext
modifications that all of my output formats use, but also have sets that
are unique to html, fo, etc., I want all of my stylesheets to point to a
common set of modifications, but also to override or add certain ones in
certain cases. From what I can tell, using the local.l10n.xml method,
you can only have one set of local gentext definitions per customization
layer. I can imagine a way to do this with entities, but it seems
cludgy.
