That works, but it's cumbersome. ;-) You can create indexterms
(semi)automatically:

 http://doccookbook.sf.net/html/en/dbc.structure.adding-indexterms.html

I used this approach and it helps to create consistent index entries.

What I left out was the following:

Individuals

However, I have found that this does not work with all versions of the
docbook stylesheets.
