Does Firefox startup if you directly double-click a .html file?  The one
dqsd is executing is in your temp folder and is called DQSDLaunch.html.
Find this file and double-click it and see which browser starts.

Could there be some formatting problem with your localprefs.js file causing
it to not load properly?

Try this -
Rename localprefs.js to take it completely out of the equation and set
launchmode=1 (not 2) in preferences.js

/* Browser launch mode
*  0=use IE
*  1=use default browser
*  2=auto (launches in IE directly if IE is default browser) [default]
*/
launchmode=1;

Yeah, I can see that this must be a windows problem. I'm really at wit's end
and may have to accept defeat. I had already tried manually associating
firfox with .htm, .html, and url:blah blah... That didn't work. The
reinstalling didn't work either! What I don't understand is that every other
application I have accepts firefox as the default. When I click on any link
or shortcut, even within Outlook, it fires up firefox ;-)  And, like I said,
the google deskbar sends its searches to firefox. Heck, even the windows
address bar opens firefox! So, although I accept that it's a windows
problem, I just don't understand how no other applications seem to fall prey
to this windows weakness... I don't want to suggest it's a bug in dqsd, but
maybe dqsd should develop some way around this issue for those of us who
can't get it working. Why couldn't there be a preference for the path to the
browser?

I dont think this is a DQSD problem but a failure of windows to handle the
default browser properly... I have had similar problems in the past..
Reinstalling DQSD wont fix the problem because the problem is upstream of
the toolbar.  I installed Firefox.  Fired it up (no pun intended :-) ) and
set it to the default browser... dqsd worked with it just fine..I then
started up IE over and over, and it *never once* inquired about setting
itself to the default browser, even though it is in the internet settings to
ask..  I started Netcaptor and it too never prompted me to set it to the
default browser (no surprise since Netcaptor is based on IE).   I loaded
setbrowser and forced the default to Netcaptor.   Still the default didn't
change.   I tried a couple of searches and I got script errors a couple of
times and on the third or fourth attempt, VOILA! I was prompted to set my
default browser..  I accepted the change and now the default is Netcaptor
again...

Clearly there is a bug in windows that it doesn't detect default browser
settings all the time.

I would mess with the browser settings and try to get windows to recognize
the changes.  DQSD merely writes out an html file and executes it.. It is up

I have had this problem as well and posted to it previously on the list 
(albeit for Firebird and not Firefox). I recieved the same responses you 
did (i.e. those listed in the documentation and which I had already 
tried). Nothing worked. As of today the problem is still unresolved.

I'm so jealous that it's working for you guys! Yeah, I used setbrowser, and
that didn't work, then I also tried just resetting it through the firefox
preferences. No dice... I haven't tried the user.js, but I don't think that
will help. I have tabbrowser extensions, and that should theoretically make
new searches open in new tabs. I just tried the new google deskbar, and it
has no problems sending searches to firefox tabs. But, I MUCH prefer DQSD...
So sad!

Default Browser under Tools -> Options -> General to reset Firefox as the
default browser?
I also made a file called user.js in the profile directory that contains one
line:
user_pref("advanced.system.supportDDEExec", false);

which causes each search to open in a new tab.
