I'll try to remember do that for *you*, but the default behaviour for
someone whose preferences I *don't* know (or don't remember) would
always have to be to include them in my reply, as discussed in the above

Tried it. Does not work because she has to re-enter the receiver's e-mail 
address. At present she just opens the mail message, modifies the subject line 
and clicks on save.

Not really, no. Read just the first part where it sets out the pros and
cons, and that's a simple technical assessment of the options, and the
various types of users that you might need to consider.

While there's a lot of nonsense in there (like the bit about "most
people post with a munged address..."), there is *one* grain of sanity
where it says "Please respect the right that these people have to decide
what goes into their inbox".

If they receive both copies because you keep them in Cc, they have the
*option* to automatically remove the "duplicates" so they only have one
copy.

But if you *remove* them from Cc they might not receive the message at
all. You have then excluded them from the discussion that you *know*
they were trying to take part in.

If you want to respect people's right to decide, you need to give them
the *option*. It's easier to remove a 'duplicate' mail than it is to
magically create one of our thin air when it wasn't sent to you in the
first place.

LOL. Not really.

I expect people that are subscribed to mailing lists to read postings on
that mailing list. If you're not interested in reading the posts on the
mailing list, why did you subscribe to it?

All those people CC'ing me fill my personal inbox just to make me read
their messages twice (once in my inbox, once in the mailing list
folder).
It's annoying. Combine it with writing "ASAP" in your email to me and be
sure you won't receive a quick answer from me.

It's not hard for you to remove the "duplicates" if you want to.

But if someone didn't get the message at all, it's much harder for them
to compensate for that. That's why it's polite to err on the side of
inclusion.
