Well, I kinda hacked a solution into place... Just running a cacls
against it every few hours with scheduled tasks... Not the nicest way
to do it...

 Exchange has repermissioned pickup to be tighter.  So I tried 
 changing it again (and unchecking inherit permissions from 
 parent..), and again it works for a while before breaking.
 
 Am I supposed to be doing this a different way?

What if you turn on security event logging?  Maybe that will log the change
in permissions and tell you when and what user account did it?
