I think the biggest failure regarding it is that is was considered such a mainstay that it has been improperly included as a default option in many products over many years - that many admins have just turned on without realizing the consequences of their actions. It is indeed poorly maintained, but unless you are particularly knowledged in these kinds of issues, its something that may not be easily recognized or understood on a level that would prompt swift action.
