Yes. 20 minutes to an hour.

Depends on several things.

DO an iisreset on your CAS to force it to happen immediately.

If we disable a user AD/email account in AD, is there any lagging time where 
they can still access email from the typical ipads, iphones, androids, etc?

My guess is we have to do something in EXCHANGE sync to force a disconnect as 
for example a mobile phone may always be logged in?

You can just disable the mobile access for the user from exchange end.
