There used to be a names utility that you could get from NAI. I couldn't
ever get any real info from it however. Not sure about the new ver.

Is there any way for me to find out who sent the offending attachment?

I know this can be done if they are quarantined to a database but what about
a directory?

If not - then is it possible to open the quarantine directory to see what
files are in there?

The resolve names utility functionality is built in to Groupshield 5.

I haven't the time to wade through the docs to find what info is stored
when you quarantine to directories.

No. Quarantine to a database and use the resolve names 
utility. If you think these attachments are coming froom the inside going 
out you've got more serious problems than the limitations of Groupshield. 
You need WebShield on your SMTP gateway
