J, Thanks for the direction with the markers it managed to provide a
solution.

Wish there was less trickier way to handle this in the XSL-FO standards
:-) like being able to control the page sequence with the fo:page-number /
tag.

But the problem has been address for now, and thank you very much for the
help.

You can try playing tricks with markers. Get a rough estimate
for the middle of the data pages, place a marker there, like
  fo:marker marker-class=logical-page-no3/fo:marker
and reference the marker in the footer instead of using fo:page-number.

You could also try using a post-processing application[1] like iText  
to insert additional pages.

Thanks J. Looks like we will have to play with markers. We'll give it a shot
and see what come out of it.

Clay, we don't yet have the option of using a post-processing application
(iText). We did consider it as a solution.
